This is a fully remote role — you can work from home while making a real, daily difference in the lives of older adults across California, Nevada, and Arizona. You will own care coordination for our geriatric patients — managing real-time communications, triaging urgent clinical needs, maintaining documentation integrity, and serving as the trusted point of contact for patients, families, facility staff, and providers.
